Crown Choices Group are looking for Support Workers to join our team, providing person-centred care to our clients in their own homes in and around Sheffield to enable them to live as independently as possible. You will be responsible for providing a high level of care, individually tailored to each client based on their needs and personal preferences.

You will:
- Follow their support plans
- Offer companionship and trips out.
- Help with meal and snack preperation
- Provide support with mobility
- Perform light household duties
- Assist with errands
- Monitor Medication schedules.
- Communicate with other professionals and family where needed.
- Driving is prefered to help with running errands and outings but not essential.

Previous experience in care homes and home care is essential.
Up to date Mandatory training and a valid DBS is required.
Excellent communication skills and understanding.
The ability to understand the needs of the individuals.
Compassion and caring approach towards the individuals.
