Clinical Pharmacist

Location: Bradford, West Yorkshire (Hybrid)

Salary: £27 - £30 per hour + Excellent Benefits

Job Type: Full-Time / Part-Time, Monday - Friday

The Client:

Our client is a well-established healthcare organisation, with more than two decades of experience in the NHS, primary care, health informatics, and the pharmacy sector.

The Role:

As a Clinical Pharmacist, you will play a pivotal role in enhancing health outcomes for individuals with chronic conditions within the primary care team.

Responsibilities:

• Evaluate acute medicine requests, manage discharge, and handle clinic letters.
• Conduct structured medication reviews and offer advice for patients with self-limiting conditions.
• Provide specialist knowledge on pharmaceutical matters, cost-effective prescribing, and clinical guidelines.
• Enhance communication on medication-related issues between the practice and care providers.
• Lead the implementation of safe repeat prescribing systems.
• Aid the practice in attaining crucial Impact and Investment Fund and QOF targets.

Requirements:

• Previously worked as a Clinical Pharmacist or in a similar role.
• Possess undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
• Knowledge of legal, ethical, professional, and organisational policies relevant to the role.
• CPPE Clinical Pharmacist Training pathway qualification or enrolment.
• Background in supporting acute and long-term conditions.
• Familiarity with intervention and referral pathways for preventable illnesses.
